The Care Closer to Home contract has led to an exciting opportunity to increase volunteers within ACE in Tendring. The role involves working within a multidisciplinary team that includes therapists and nurses. We are seeking to recruit enthusiastic and motivated individuals to contribute to excellent aftercare to support clients with their transition from treatment to self-management.
The aims of the Volunteer role are to increase a client's independence through support, care and rehab within the comfort of their home on discharge from hospital or on referral from a community health professional. The role includes building confidence, helping support care plans, booking appointments, signposting relevant services and groups in the local area and providing social support. We set goals to be achieved, agreed between the therapist and the client.
The Health Partner role within Tendring requires individuals who are confident, have excellent communication and interpersonal skills and enjoy working with a variety of service users of different backgrounds and age groups. It is essential that you are willing to travel around Tendring as you will be required to travel to meet with clients either at their home or within the community.
You will be responsible for prioritising and planning your own caseload and will be expected to review and support the patients to achieve their end goals, and have the knowledge to refer to other services when necessary.
Full training will be provided and due to the nature and length of the training, a commitment to the role of a minimum of 4 hours per week for 6 months is required to enable the volunteer to fully benefit from the role.
It is desirable that applicants are able to drive and have access to their own vehicle; however not necessary if a volunteer is willing to use transport.

Volunteers are given a caseload of 1-2 clients per week (can vary depending on what the volunteer wishes to do) then its up to the volunteer to speak to the client to arrange a time that suits both the volunteers and the client. Very flexible hours.
