Our link workers are offering a brand-new service to help people make the transition from hospital to home with as much support as we can. This role will involve delivering a pre-prepared food pack containing enough for a few meals. You will add some fresh items to suit and drop off at an arranged time.
This can
make all the difference to those who come home to an empty house, maybe after a
long stay in hospital and with no family or friends to help out.
